The Initial Psychiatric Evaluation is required at the beginning of a patient’s treatment. A comprehensive psychiatric evaluation is necessary for proper  diagnosis of your mental health disorder. An evaluation of a child, adolescent or adult is made based on presenting symptoms and behaviors in relation to physical, social, cognitive (thinking), emotional, or educational aspects that may be affected as a result of these behaviors. The Evaluation consists of the provider’s assessment and diagnosis of your mental health. Our provider will ask many questions that will result in a treatment plan and goal setting. Some of the questions include.

  • Describing your behaviors, how long do they last, what triggers them.
  • Describing your symptoms; both physical and mental health symptoms
  • Describing how the behaviors or symptoms affects work, school, and relationships.
  • Providing your personal and family health and mental health history
  • Completing a health history, including a description of the person’s overall physical health, list of any other illnesses or conditions present, and any current treatments

The provider may order lab tests in some cases. These may be used to find out if you have an underlying health issue that may be contributing to your symptoms.

During your initial session at Gladstreams Mental Health & Wellness, our provider works towards establishing a therapeutic relationship and strives to understand your concerns and answer any questions you may have during your assessment.

What to Bring To The Appointment.

Prior mental health records and current medication bottles, if applicable. It is also required that you complete our new intake paperwork before your appointment begins.
